Aging with Grace Services
Preferred Provider Member Discounts
To be a member of the AWG Preferred Provider Network, the Provider must agree to extend a discount to AWG members.
The discounts are as follows:
- Senior Living Communities
One time discount credited to the 3rd month rent as follows:

| 3500.00+ | $1250.00 |
| 2500 - 3499.00 | $750.00 |
| 1500.00 - 2499.00 | $500.00 |
| $1000.00 - 1499.00 | $350.00 |
- Home Care - 10% discount on services greater than 10 hours per week up to a maximum of $1250.00.
- Adult Day Care Services - Discount of $50.00 per month on services of 3 or more days per week up to a maximum of $1250.00.
- Geriatric Care Management: Discount of 10% for individual members and 15% discount for UnionPlus members off advertised rates for services.
- Senior Move Management: Discount of 10% for individual members and 15% for UnionPlus members off advertised rates for services.

The AWG Member Discount is not applicable for any individual for whom payment will be received by the Preferred Provider from any Federal or State Program, including Medicare, Medicaid or a Medicaid-Waiver Program.
