Member information is shared within Aging with Grace in order to provide members an efficient online experience. We will not disclose your personally identifiable information to third parties except to the extent necessary to provide you with a requested product or service.
We assist families by evaluating eldercare issues, educating members about the options available, the type of care needed by an elderly person including an assessment of finances and amenity preferences. Once we determine the individual's needs and preferences, we identify senior care providers which we determine could potentially meet those needs and preferences and assist the member with coordinating and facilitating the needed services. The eldercare specialist provides this information to the member and develops a plan of action for the member to follow. We also will forward the individual's name and pertinent information to the provider(s) along with a request for direct contact from the provider(s) with the member's permission or by request.
To the extent you are notified of uses of your personal information which are different from those described in this Policy Statement during your enrollment in any particular service, or program, the uses of which you are specifically notified will govern that service or program.
